My husband, Zachary, lives at Avantara Evergreen Park. He is a veteran who goes to Jesse Brown VA Hospital for specialty care, and his doctor is very impressed with the wound care he receives at Avantara. The wound care team does a great job keeping the wounds clean as they heal. I’m grateful to their team, especially Kenera, Mary, Sherri, Marletha, Rainy, Stephanie, Shalonda, Avis, Vera, Celelita, Gloria, Penet, Tony, and Betty, for their genuine care to my husband and for being accommodating and pleasant to our family. They all go above and beyond to ensure he gets everything he needs, and seeing them tend to his needs means a lot. I will be forever thankful to everyone at Avantara for caring for my husband.

I am a retired registered nurse who worked at the University of Chicago Hospital for 36 years. I was a patient at Avantara Evergreen Park from 2022 to 2023, and I received excellent patient care as I recovered from knee surgery. I was treated with dignity and respect throughout my stay. I would like to recognize your team for their exemplary care, especially Ashley (LPN). She is an exceptional employee. She went above and beyond the call of duty and took a personal interest in her patients and their families. She made my stay as comfortable as possible. She responds to residents positively and reassuringly. She demonstrates a high level of pride in her profession, and she is skilled in her daily patient care. She was very good to me. She is an asset to your facility and the residents and families that she serves.

My brother has been staying at Avantara Evergreen Park for two weeks now. He was having difficulty walking after major surgery, so we wanted him to stay in an excellent place with in-house dialysis. The staff is always cordial and happy to assist. My brother has a great stay with no complaints. He is very happy and content. He feels safe, making a lot of progress, and moving around better. When I came to pick him up once, they made sure to have my brother ready downstairs, so I didn’t have to go all the way to his room. They’re so thoughtful and considerate! Great service! The facility is white-glove clean, spotless! I worked in a hospital, and I know it’s important to give a shout-out to a team for doing a good job. I also appreciate Ron (hospital liaison) for being accommodating and answering our every question. He was a great help in placing my brother in Avantara, and we are looking forward to discussing more about my brother’s long-term placement after his discharge.

I came to Avantara Evergreen Park for physical and speech therapy after a stroke. When I first arrived, I could not walk on my own and needed the assistance of a wheelchair. The physical therapists created a plan to help me regain my strength, and now I can walk from the gym to my room with only the assistance of a walker. The speech therapist also did a great job. I was having trouble talking, and now I can speak better than ever. The staff at Avantara is excellent. Thank you for everything you did to help me recover. I am excited to return home soon!

Avantara Evergreen Park was clean and organized. Shalonda, Kelly, Williams, and the entire nursing staff were pleasant and knowledgeable. I also appreciate Carol (CNA) and Sherrise (office manager) for accommodating and genuinely caring for my dad from the moment we got to the facility. Thank you, Avantara, for taking excellent care of my dad!

Veeda, Henrietta, and the rest of the wound care staff were pleasant and friendly. I’m thankful for their care and compassion, which helped my mom heal better. Bob (CNA) took extra care in bathing her. Rainey (social services coordinator) and Loretta (escort) went the extra mile to help us get services and other items needed when we went home. I’m grateful that they value their jobs and give the guests the love and attention they deserve.

When my grandmother needed rehabilitation after her leg amputation, our family wanted to make sure she was somewhere she could heal, feel supported, and continue making progress.
Unfortunately, our first experience at another rehabilitation facility was not a positive one. She felt uncomfortable, isolated, and struggled to receive the kind of support she needed. After additional medical complications and another hospital stay, we were determined to find a better option.
That’s when she came to Avantara Evergreen Park.
From the beginning, the difference was noticeable.
One of the biggest challenges my grandmother faced was adjusting to life after her amputation. She needed to rebuild strength, improve her mobility, and learn how to safely move between her wheelchair and bed.
The therapy team worked closely with her every day to help her regain those skills.
When she first arrived, many simple movements were difficult. Over time, she became stronger, more confident, and much more independent. One of the biggest improvements we saw was her ability to transfer safely and move more easily using her remaining leg strength.
The progress she made exceeded our expectations.
After our previous experience, we hoped for improvement, but we were honestly surprised by how much mobility and strength she regained during her stay.
What stood out most was how the staff connected with her on a personal level.
Spanish is her primary language, and the team made a genuine effort to communicate with her in Spanish whenever possible. That meant so much to our family. It helped her feel comfortable, understood, and more connected to the people caring for her.
The staff didn’t just focus on her medical needs—they took time to learn about her as a person.
They talked with her, listened to her, and made sure she never felt alone.
For my grandmother, therapy quickly became her favorite part of the day. She looked forward to seeing the progress she was making and remained focused on her goal of returning home stronger than when she arrived.
Beyond therapy, she enjoyed the overall atmosphere of the facility. The activities, dining services, and daily interactions helped create a sense of community that made her feel welcome and supported.
Most importantly, she felt at home.
As a family member, that’s what gave me the greatest peace of mind.
When your loved one is recovering from a life-changing event, you want to know they’re being treated with kindness, patience, and respect. At Avantara Evergreen Park, that’s exactly what we found.
I would absolutely recommend Avantara Evergreen Park to other families. The staff is compassionate, the therapy team is outstanding, and they create an environment where residents feel valued, supported, and encouraged to succeed.
For our family, that made all the difference.

After my surgery, I was in a lot of pain and having trouble standing, balancing, and walking. When I came to Avantara Evergreen Park, the physical therapy really helped me get stronger and regain my mobility. By the time I left, I was walking with my walker and doing so much better.
I’m grateful for the care I received at Avantara Evergreen Park. The staff was kind and helpful, and I felt like they truly wanted to see me improve. My stay helped me get back on my feet, and I would recommend Avantara Evergreen Park to anyone who needs rehabilitation and support.
